Answer:

The file's inode must change for the plug-in to re-read the file from the beginning (even if the Rewind option is enabled).

A simple replacement of the contents (i.e. cat file.txt > log.txt) will not trigger the re-read. But a move or copy (i.e. cp file.txt log.txt) will trigger the re-read of file from the beginning.
